The Wellness Impact Program
The Wellness Impact Program is our approach to providing quality, comprehensive care for our residents.
This program offers ongoing comprehensive Health Services Assessments, along with personalized service plans and health care coordination by working directly with personal physicians and other health care providers.
We can also offer in-house physical, occupational, and speech therapies. Lastly, our pharmacy partnership ensures a high-quality medication management program.
Through the Bridge App, families can stay up to date with their loved one’s wellness journey and can even connect with our wellness team for more insight and support.

Wellness Impact Frequently Asked Questions
Health Services Assessment actively tailors care and service to each resident’s individual needs.
Based on the Health Services Assessment results, residents are charged a monthly Level of Care fee corresponding to their required services. Residents only pay for what they’re using, protecting peace of mind for them and their families.
Our assessment considers the number of staff and the amount of time to deliver all required services. It is associated with the following general areas:
- Assistance with daily living activities
- Health care services
- Health care planning & coordination
- Medication management
- Nutritional requirements
We complete an Health Service Assessment:
- Prior to move-in to our community.
- 30 days after physical move-in.
- Every 90 days for memory care residents.
- Every 180 days for assisted living residents.
- After any health care status change.
As a matter of policy, a member of the management team coordinates a care plan meeting at the time of the routine Health Services Assessment.
We may also hold a care conference in certain circumstances. Care conferences are an opportunity for families to participate in their loved one’s care-planning. Additional care conferences may be held when there are changes in a resident’s condition.
Levels of Care and corresponding monthly Level of Care fees can change based upon changes in a resident’s condition—however, frequent fluctuation is not common.
If the monthly Level of Care fee changes as a result of an increase or decrease in resident care, a member of the management team will contact the resident and/or family member to review the situation.
